Set Up a Screening Appointment

    Step 1: Intake/Pre-Registration

    The purpose of this step is to create a student file in the preschool office.

    Parents and/or legal guardians are required to complete the Parent Input, Registration and Child Information Packets found here. They also must provide the following documents for pre-registration:

    • Birth Certificate
    • Immunization Record
    • Proof of Residency*
    • Photo ID (Driver's license, state ID or passport)
    • Hearing and vision results (If available and dated within one year)
    • Any outside therapy notes (If your child is receiving speech therapy, occupational therapy, counseling or other services through an outside, non-school agency)

    *Please note, appropriate proof of residency is a valid driver's license, utility bill dated within the last month (30 days), paystub, bank/mortgage statement or lease agreement (signed by BOTH the landlord and the tenant). If the parent(s) name is not on the proof of residency because they are living with someone else, the parent must fill out an "Affidavit of Residency Form". The form must be notarized and turned in at the time of Intake/Pre-Registration.You will also need to bring the utility bill, mortgage statement or rent statement for the person listed on the Affidavit of Residency Form. The form may be obtained from the Search to Serve Office.

    Once Intake/Pre-Registration is completed, we can proceed to Step 2.

    Step 2: Schedule Screening

    Once all Intake/Pre-Registration paperwork has been submitted, a member of the Search to Serve team will contact you about the date of the next available screening for scheduling. As a reminder, we are unable to schedule a screening until ALL the requested paperwork and documents have been received and processed.

     What to Expect at a Screening Appointment

    The parent(s) and/or legal guardian(s) will discuss the child’s development with the special needs preschool screening team.

    The child is screened in the following areas of development:

    • Communication skills
    • Reasoning/thinking skills
    • Self-help/adaptive skills
    • Personal-special skills
    • Motor skills

    The staff will review the results with the parent(s) or guardian(s) to determine next steps.
